Registering with the practice

The practice operates a policy of non-discrimination on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition from patients wishing to join the practice. Further information about our practice can be found at

For patients wishing to register you are required to live within Gadhvi Practice catchment area. No proof of ID or proof of address is needed. Paper registration forms are available from the Gadhvi Practice reception, but registering online is recommended.

How to register Online

To register at Gadhvi Practice all new patients are required to complete two electronic registration forms (see below). Once both forms have been completed please sign and return by email to On receipt of your completed registration forms Gadhvi Practice will acknowledge your acceptance to the practice, you will also be asked to make an appointment for a new patient check.

When registering online you do not have to provide ID or proof of address.
